JOB SUMMARY: This position is vital to the COVID-19 response and serves as an advocate for community members as well as a liaison between the community and health and social service resources. Develops and maintains a close relationship with the community and clearly understands the barriers and constraints the community faces in accessing needed community resources. Provides COVID-19 community outreach, education and support in a manner that culturally supports the community being served. Facilitates access to services, offers social support, helps people get the care they need, offers translation services to community members impacted by COVID-19.

RECOMMENDED QUALIFICATIONS:
Course work from an accredited college or university in Social Work, Psychology, Public Health, or a related field and three (3) years of experience providing community health worker services to a comparable client base.

A current Utah Driver's license and reliable transportation is also required

Preference will be given to applicants who are multilingual in English and the language(s) of the community being served.

ESSENTIAL FUNCTIONS:
Interacts with community members and connects them with needed resources.
Promotes services within the community by establishing relationships with local schools, service providers, community groups, churches, age-specific programs, and others.
Develops cultural awareness material and provides education and training to individuals, small groups and community partners.
Provides information, education and outreach on COVID-19 issues and teaches community members ways to mitigate its spread.
Explains COVID-19 related medical terms and concepts regarding exposure and symptoms as well as preventative measures, including isolation/quarantine, social distancing, environmental surface cleaning.
Networks, develops and maintains partnerships with community support agencies.
Helps community members identify needs they may have for social support during periods of self-isolation or self-quarantine.
Provides advocacy to address health disparities at the individual and community levels.
Gathers and provides statistics on numbers of community members served and community health worker outcomes for Salt Lake County Health Department
Gathers feedback about additional community needs and gaps in services through focus groups, surveys, and individual discussion. Works with community agencies to help solve these gaps.
May schedule appointments with health/social services agencies for community members.
May enter the COVID-19 related information into the state's public health management system (EpiTrax).
