A 6 week self-management class for people 60+ who are living with a chronic condition
A 6-session evidence-based workshop designed for those dealing with a chronic or limiting health condition. Participants learn self-management techniques and skills needed in the day to day management of any type of ongoing health condition.
Topics include:
Effective Communication with Healthcare Providers
Relaxation
Healthy Eating
Sleep
Physical Activity
Breaking the Symptom Cycle
Problem Solving/Action Planning
The program has also been proven to be effective for caregivers.
