A free, six week program designed help manage chronic health conditions. Class is held for 2 1/2 hours, once a week, for six weeks. Focus on problem solving, decision making, action plan, and confidence building.
A 6-session evidence-based workshop designed for those dealing with a chronic or limiting health condition. Participants learn self-management techniques and skills needed in the day to day management of any type of ongoing health condition. The program has also been proven to be effective for caregivers.
Free to older adults and caregivers.
